Individual Case Management & Assessment

When families are directed to the Center for emergent needs

The Center sees families and assesses their needs, formulating a family goal approach and case plan. Family advocates are also available. Many of the services are at the Center, and other times we triage the need to one of our community partners. Needs can include:

  • Medical/mental health issues

  • Substance abuse

  • Housing

  • Utilities

  • Parenting concerns

  • Youth concerns

Information & Referral Services:

This is one of our key services and sometimes related to individual case management and assessment services. Individuals may come in and request specific services; trained family advocates and social workers will provide information and referrals. Sometimes the needed resources do not require case management and assessment. Many times, we provide the community member or family with a list of churches, food banks, Spanish-speaking advocates for immigration issues or other resources.
